certified and have been caring for the members of our community since

The goal of Butte Home Health
is to help the patient achieve the greatest possible level
of health and comfort... at home.
Reasons for home health:
Home Health Care provides an alternative to extended hospitalization.
Recovery is often speedier and more complete in the home
environment.
The patient is often happier and more comfortable in their
own home.
It is usually less costly than other alternatives.
For the purposes of home health, a "Home"
is defined as:
Personal Residence
Guest Home
Retirement Home
Residential Care Facility
Home Health care uses a team approach that includes coordinated care
and support systems designed to maximize independence at home.
Butte Home Health & Hospice can provide
the following services:
Home Health Nurse
The Home Health Nurse plans and provides
nursing care in the home, such as IV Therapy, Wound Care, Injections,
Nutritional Teaching, Monitoring, and Patient/Family Instruction.
Registered Nurses and Licensed Vocational Nurses (under the supervision
of RN's) can provide the following services:
| • Skilled assessments |
• Instruction and monitoring of medications and disease
process |
| • Symptom control |
• Instruction and monitoring of treatments (i.e: wound
care) |
| • Injectable medications |
• Nutrition and special diets |
| • IV medications |
• Pain management |
| • Foley catheter management |
|
* An Administrative Nurse is on-call 24 hours a day, 7 days a week
to direct the on-call home care nurses for after hours needs.
Home Health Aides
* Home Health Aides provide personal care for the patient, including;
Bathing, Skin Care, Shampooing, and Bed Positioning.
* Therapeutic exercises as prescribed by a licensed therapist.
* Assistance in maintaining a clean, healthful home environment for
the patient.
* Home Health Aides are on-call 24 hours a day and are activated by
the administrative nurse in the event of a patient or caregiver crisis.
Therapy
Physical Therapy
* Develops and restores function of limbs and muscles. A hands-on
approach to enhance healing and decrease pain.
* Instructs patients and family in rehabilitative activities to achieve
maximum independence.

Occupational Therapy
The following services can be provided:
*Skilled evaluation
*Training and exercise
* Developing a program to promote independence, including:
| • Dressing |
| • Personal Hygiene |
| • Meal Preparation |
| • Home Maintenance |
* Identification and training in the use of adaptive equipment to
promote independence with activities of daily living!
Speech Therapy
Provided by a Licensed Speech Pathologist to help regain or maintain
physical or mental functions.

| • Swallowing training |
| • Instruction in smelling |
| • Precautions |
| • Cognitive skills training |
| • Memory skills training |
| • Communication techniques |
Medical Social Worker
| • Assist with long-term plans, obtaining financial aid
and other resources. |
| • Assist with coordination of community resources. |
| • Assessment of social and emotional factors impacting
the patient's care. |
| • Counseling for the patient. |
| • Alternative living situation. |

Initiating:
| • Any individual, physician, or concerned person may
request services by calling Butte Home Health. |
| • Following a request for home health care, the patient's
physician will be contacted for approval of services. |
