  • Home health benefits bring skilled nursing, therapy, or even medical social work visits to your home that help you recover, stay safe, and remain independent. All services are done under the supervision of your doctor.<\/li>\n
  • Home health can help you transition home after a hospital stay or after a stay at a rehabilitation facility. Home health can also be ordered by your doctor if your health condition or medications change. Our caring professionals visit intermittently (often two or three times a week) and focus on helping you remain independent at home. They will teach you about your condition, help you understand your medications and ensure you take them at the right time and in the right amounts, working to help you with your nutrition needs, keeping you safe from falls risks, and get back to doing your normal activities around your home.<\/li>\n
  • Home health is highly skilled, delivered by experienced professionals, and provides a personal plan of care to get you going again, safely and independently while keeping you where you want to be most – in the comfort of your home.<\/li>\n<\/ul>\n

                  Initiating:<\/h4>\n

                  • A patient, caregiver, family member, physician, or concerned person may request services by calling Butte Home Health.<\/li>\n
                  • Following a request for home health care, the patient’s physician will be contacted for approval of services.<\/li>\n
                  • You can initiate services by calling (530) 895-0462 during office hours: 8:30 AM to 5:30 PM, 7 days a week<\/li>\n<\/ul>\n
