We’re here for the community during unprecedented times. Our team of nurses, therapists, aides, and social workers are working hard during this crisis to keep the most vulnerable in our community safe at home and out of the hospital, where the most serious will be treated. Agency wide, we are taking the following actions:


• Abiding by CDC guidance, and following closely the communication from our local public health department
• Screening our patients each visit for COVID-19
• Daily screening of our staff
• Our in-person patient team meetings have moved to virtual to support social distancing
• Providing proper PPE (personal protective equipment) with every visit in our patient homes
• Providing telephone or virtual nursing and therapy visits for our patients, when appropriate
• Weekly general wellness calls to our patients to ensure they have the supplies and information they need to stay safe at home
