Butte Hospice is hosting an educational presentation on the 5 Wishes Document. 5 Wishes is a form that helps individuals and families make important decisions that are meaningful and understandable to them. It covers personal, spiritual, medical and legal wishes all in one document. Advanced care planning relieves families and caregivers from the difficult position of guessing your wishes.

Please join us on Wednesday November 14^th, 12:00 pm at our office which is located at 10 Constitution Drive in Philadelphia Square in Chico. Lunch will be served, and all attendees will receive a free copy of the Five Wishes document.
